Founded as one of the first home health providers in the area, St. Joseph Home Health Care became Trinity Home Health in 2007. Trinity Home Health strives to meet each patient’s medical, social, and spiritual needs through a comprehensive array of quality, cost-effective services designed to maintain independence at home. In providing these intermittent services, we involve the entire family in the complete scope of our services. In addition, we offer services to those who need and qualify for services regardless of socio-economic status. We are dedicated to promoting the physical, emotional, and spiritual well being of our patients and all who come in contact with our agency. Trinity Home Health serves the following 16 counties in Georgia: Richmond, Columbia, McDuffie, Warren, Lincoln, Wilkes, Taliaferro, Glascock, Hancock, Burke, Jefferson, Screven, Jenkins, Emanuel, Johnson and Washington counties. Additionally, we serve three South Carolina counties to include: Aiken, Barnwell, and Edgefield counties.
Admission to this agency can only be made under the direction of a physician and is based on the patient’s identified care needs, homebound status and the type of services required that we can provide directly or through coordination with other organizations. If we determine that our services cannot meet the needs of the patient, we will not admit them or will not continue to provide services to the patient.
The Skilled Nurse is an RN or LPN with training and expertise in providing care in the home. The nurse communicates frequently with the patient’s physician to update the plan of care. Services may include:
Physical, Occupational, and Speech Therapy services are provided by a licensed therapist or licensed therapy assistant under the direction of the therapist. The therapist will work to improve the patients’ motor abilities and communication skills so that patients can function in their home and community as independently as possible. In the event that a skilled nurse is not a part of a patient’s plan of care, a licensed therapist will be the case manager.
The Medical Social Worker provides short-term counseling services, referral to and coordination with community resources and assistance with planning for present and future care needs.
Home Health Aide Services are delivered under the supervision of a registered nurse or licensed therapist. Our aides have had experience caring for people in need and have been carefully chosen to provide care for the patients of our agency. An aide sees a patient when there is a specific short term need for personal care assistance in the home that might include a bath, mouth care, hair care, nail care, shaving, linen changes, etc.
